Bad day at the office?

Tug Titan sank off Miami. She fell to human error. The line was suppose to be pan in, underwater pics shows pan out. Engine room doors where open. They had been told over and over to keep them shut. This is the second time she went down and second time with engine room doors open. Jan of 2004. Last time. Artical from tugboats@yahoogroups.com.
